Community Health Centers: The intersection of public health and primary care

Community Health Centers (CHCs), also known as Federally Qualified Health Centers (FQHCs) are at the intersection of public health and primary care. CHCs are a tool for public health professionals to connect vital resources for patients and communities who are the most vulnerable and most underserved within the community. During this session we will be speaking about the structure of the Community Health Center program and its roots with the Civil Rights Movement. We will dive deeper into Nevada’s CHCs, sharing what they do, who they serve, and how they care for patients through innovative practices connecting medical care with the social determinants of health.  Public health leaders will learn about how they can use health centers to address identified community health needs through population-based care. As public health policy pushes for improvement on chronic disease management, FQHCs are the ideal community vehicle that can drive those efforts onto the patient level.

 



Public Health Perspectives is a free online webinar series designed to showcase programs impacting public health. PHP discusses program design, lessons learned, and creative solutions with public health professionals around the nation. Each PHP webinar takes approximately 1-hour to complete. 

PHP Webinars are hosted live on the second Friday of each month at 10am PST.  

To join the live session, please enroll in the course before 10am PST on the day of the webinar. All PHP sessions are recorded. If you enroll after the live webinar session has ended, the course will contain a video recording of the live session you can watch instead. Please note, it may take up to one month for the recording to become available.


Learning Objectives:

At the end of the webinar, participants will be able to:

  1. Identify at least three unique characteristics of a FQHC and why FQHC growth in Nevada has been substantial over the past decade.
  2. To provide a framework for the audience to understand how FQHCs address social determinants of health, how they align with public health and address community needs and why they are a laboratory for policy in healthcare.
  3. To inspire the audience to understand the significance of FQHCs.

About the Speaker


Reema is the Performance Improvement Program Manager at Nevada Primary Care Association. She provides education, technical assistance, and training on quality and performance improvement strategies for community health centers and supports community health centers with workforce development. Ms. Naik is also a coach for the Health Center Control Network in Arizona, which is a network of 28 health centers that work together to improve access to care, enhance the quality of care, and achieve cost efficiencies through the provision of management, financial, administrative, technological and clinical support services. She is also a Certified Lean Six Sigma Green Belt.
